Financial & Payment Information

We pride ourselves for delivering services at competitive rates suggested by the Ontario Dental Association’s fee guide. Despite the fact Dr. Wallace is a specialist in Oral and Maxillofacial Surgery and board-certified in both Canada and the United States, the majority of procedures are completed in line with the general dentistry fee guide.

During your consultation, Dr. Wallace will provide you with an estimate and pre-determination to submit for those with insurance coverage. For those with insurance coverage, we strongly suggest you know your policy information so that we can submit your predetermination on the day of your consultation to get a clear idea of what amount is covered. You are fully responsible for your fees as with the privacy act, we cannot discuss any variance in coverage directly with your insurance, or on your behalf.  Please see our first-time patient link for other items of interest that will help you through your first appointment. Please see more detailed notes below related to surgeries requiring general anesthetic in the office or in the hospital below.

For your convenience, the office accepts Cash, Debit, Visa and Mastercard.

If you have questions regarding your account or billing, please contact us at (519)884-2500 to quickly resolve any issues.

Is your surgery taking place in our office?

The extraction of wisdom teeth is a procedure most commonly performed within the office, either under a general anesthetic, or by intravenous sedation. If a general anesthetic is used, the fee for the professional anesthetic service is set by the local medical anesthesiologists and varies depending on the length of the procedure. At the time of your consultation an estimate will also be provided for the anesthesia portion of the procedure.

Because this portion of the fee is billed by the anesthetist, a qualified medical specialist, we request that you bring a separate cheque or cash for the anesthetist dated for the day of the surgery. You will be made aware of the name of the anesthetist and the total cost of the anesthetic on the day of the surgery. Please note that there is a $25 fee for NSF cheques.

While the Ministry of Health does not cover the anesthetist fees, your dental insurance may cover a portion of the total cost and a predetermination is advised following your consultation. Insurance often reimburses a majority of the cost, however, the coverage will depend on the deductible for your plan, the annual maximum, and the year and type of fee schedule your plan uses.

Is your surgery taking place in the hospital?

In special circumstances, a patient may require the surgery to take place in the hospital, in which case the Ministry of Health will cover the cost of the hospital bed. However, the payment for removal of teeth may be the patient’s responsibility. Typically, the only individuals who may qualify for Ministry of Health / OHIP coverage to remove teeth are those patients with;

  • Uncontrolled Diabetes

  • Cardiac patients with significant valvular disease

  • Uncontrolled high blood pressure

  • Uncontrolled epilepsy

  • Uncontrolled asthma or emphysema

  • Haemophilia

  • Cancer

These situations require prior approval for payment from the Ministry of Health.

For those who do not qualify for OHIP coverage, once a hospital surgery is booked, payment is required 2 weeks prior to the surgery date. Your insurance forms will be mailed to you on the next business day following surgery. Please note any refund as a result of a difference between the estimate and the amount will be made following surgery. If there is a balance owing between the estimate and the amount billed, we will contact you with the amount the following week. Upon booking, you will be provided with detailed information regarding payment of the anesthesia fee.
