The following forms are provided in-person at your consultation. We’ve made these forms available online to save you the hassle of returning to our office if any are lost.
Common for all Procedures
Covid-19 Screening Form (You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it)
General Consent Form (to be completed by yourself, mandatory for all surgeries and procedures)
General Anaesthetic Procedure
Pre-Anaesthetic Release of Responsibility (to be completed by yourself, for all surgeries to be conducted under a general sedation)
Pre-Anaesthetic Questionnaire (to be completed by yourself )
Pre-Anaesthetic Instructions (to be reviewed by yourself for surgeries to be conducted in our clinic under a general sedation)
Non-General Anaesthetic Procedures
IV Sedation Instructions (to be reviewed prior to surgery under intravenous (IV) sedation)
Oral Sedation Instructions (to be reviewed prior to surgery when a pill will be used to provide moderate sedation)
